The COVID-19 vaccine is recommend for everyone 6 months and older, and additional doses are recommended for immunocompromised individuals and those 65 years and older. Before booking an appointment for the COVID-19 vaccine, it’s important to know current health and safety recommendations from the CDC.‡ Please have the following personal information ready before scheduling an appointment: date of birth, physical address, email address, Medicare ID number (if applicable), state and county, and emergency contact name and number. In addition, please be ready to answer questions about your allergies and long-term health history.
