We'll deliver your diabetes testing supplies right to your door with no charge for shipping. And if you ever have a question, just give us a call at 1-800-PUBLIX-RX.  The Publix Pharmacy Direct Care staff can answer your questions about your medications, diet, exercise regimen, and more.

You may sign up either by calling your Publix Pharmacy Direct Care Call Center at 1-800-PUBLIX-RX or follow these three easy steps:

1. Complete the Enrollment Form with the appropriate personal, insurance, and medical information.
2. Sign and return the Acknowledgments and Authorizations for Assignment of Benefits and Use/Release of Medical Information (if eligible for Medicare) and/or Credit Card Authorization Form (for payment of applicable co-payments, co-insurance, or deductibles).
